For those New Players who are born in Canada, Live in Burnaby and have never registered with another minor hockey association before.

Before beginning the registration process, please:

• Confirm you reside in the City of Burnaby’s residential boundaries
• Review the BMHA Fees and read/accept BMHA’s Refund Policy
• Create a Hockey Canada ID - instructions can be found here

As part of the registration process, you will be asked to:

1. Fill out the PCAHA Registration Form
2. Provide a copy of players birth certificate
3. Provide two (2) proofs of residency – ALL proofs of residency must be current and show your full address:
   • Two proofs needed that show Mother or Father's name and address;
   • ONE of the following statements:
     • BC Hydro
     • FortisBC
     • Phone
     • Cable
     • complete lease agreement; and
   • ONE of the following statements:
     • Utility/water
     • Bank
     • BC Medical
     • Credit card
     • Car insurance, with copy of driver's license
     • School registration, confirming address
4. Registration fee: Credit Cards (Mastercard or Visa) are our only accepted method of payment. Unfortunately, no cash or cheques will be accepted.

REGISTRATION PROCESS

***STEP 1:  Before you can register, you MUST have a Hockey Canada Identification Number (HCR#). A HCR# is issued by Hockey Canada and can take up to 5 days to receive.

Hockey Canada will send your HCR# to the BMHA Registrar and our Registrar will contact you with your new HCR#.

After you received the HCR# for your athlete, the BMHA Registrar will contact you for the necessary birth certificate and residency documents and then you can complete your registration.

If you have received a HCR# you may proceed to STEP 2.

***STEP 2:  After you have been contacted by the Registrar and received a HCR# from the Registrar, please complete your Microhockey registration.
