There are various forms and documents that are required as part of the Registration Process. They include:

BMHA Coaches/Managers' Forms & Documents
- BMHA Managers Manual (updated September 2026) - BMHA has a manual to help our team managers with the season.
- Room Bookings Request Form - to book a meeting room at one of the arenas, please allow 7-10 business/working days to process.
- Referee Payment Schedule - this will help track referee payments throughout the season, all unused moneys must be returned to the Treasurer at the end of the season.
- Team Jersey Inventory - Start of Season - this will help inventory your team jerseys at the start of the season.
- Affiliates Form - U11 - please use this to identify and roster affiliate players for your team.
- Affiliates Form - U13 to U21- please use this to identify and roster affiliate players for your team.
- BMHA Conduct & Incident Report Form - please visit the Bylaws, Policies & Procedures page to view the Policy, SOP and access the online form.
- BMHA External Instructors & On/Off-Ice Activity Notification Form - please use this to notify the Association of the team's use of third-party/non-rostered instructors or off-ice/non-regular programming.
